Checklist

  • 1. What to Bring to Your First Appointment — Government-issued photo ID (driver’s license or passport), current physical insurance card, and form of payment for your required co-pay or co-insurance.
  • 2. Prescription & Supplement Bottle List — Please bring all active medication bottles or a detailed typed list showing medication names, exact milligram dosages, and prescribing physician to ensure safety.
  • 3. Prior Diagnostic Lab Work & Imaging — If you have had bloodwork, EKG, DEXA bone density, or ultrasound imaging within the past 12 months, bringing copies avoids unnecessary duplicate testing.
  • 4. Arrive 15 Minutes Early (or Complete Online) — Complete digital registration via our Patient Portal link sent by SMS, or arrive 15 minutes prior to your calendar time for in-person check-in.
